Meet Ashley Ramos
​​Exciting Addition To Our Social Work Program​

At Genuine Health Group, we celebrate exceptional programs and the individuals who drive their success. Today, we are excited to highlight our Social Work Program, one of the most vital aspects of our comprehensive care approach, and introduce Ashley Ramos, the newest social worker on our dedicated team.

 

Our Social Work Program provides a network of resources across all Genuine Health At Home programs, including CCM, TCM, and HEP. Our team plays a vital role in coordinating identified needs using federal, state, and local community resources, and providing emotional support and education. Social workers like Ashley thoroughly assess members’ needs and connect them with vital community resources. Their work is crucial in assisting members with access to necessary services, ensuring eligibility, and maintaining optimal health. 

 

Ashley’s comprehensive background and commitment to supporting others make her an outstanding addition to the team. Ashley holds a Master of Social Work focusing on mental health and substance abuse, complemented by a bachelor’s degree in child development.

 

Ashley, we are excited to see the positive impact you will continue to make through our Social Work Program. About Our Health Equity Program
​​
In our Health Equity Program, the Master of Social Work (MSW) plays a pivotal role in addressing and mitigating health disparities among our patients. The MSW’s responsibilities are multifaceted and essential for ensuring comprehensive patient care and include:

Biopsychosocial Assessments Conducts thorough biopsychosocial assessments to evaluate the physical, psychological, and social factors affecting a patient's health.

Identifies each patient's unique needs and challenges, ensuring a holistic approach to care.

Community Resource Navigation Identifies and connects patients with relevant community resources and services.

Facilitates access to housing, food security programs, transportation, and other vital community-based supports.

Addressing Social Determinants of Health (SDOH) Provides targeted assistance with issues related to social determinants of health, such as income, education, employment, and social support.

Works to remove barriers that impede patients' ability to achieve optimal health outcomes.

Advocacy and Support Advocates on behalf of patients to ensure they receive the necessary services and support.

Offers emotional and psychological support, helping patients navigate complex healthcare and social service systems.

Collaboration and Coordination Collaborates with healthcare providers, patient care navigators, and other members of the healthcare team to coordinate comprehensive care plans.

Ensures seamless communication and continuity of care across different services and providers.

By integrating these key functions, our MSW in the Health Equity Program significantly contributes to improving patient outcomes and promoting health equity within our community.
