Leveraging Social Determinants of Health

 

Social determinants of health, or SDOH, include food, housing, security, and transportation – and these can significantly impact the physical and mental health of patients. Capturing this data can help improve patient outcomes, while also improving barriers related to housing and transportation, health literacy, social support, food access, and financial assistance.   

 

With increased burden on healthcare providers and their teams, it can be challenging to consistently identify patients who need assistance with additional community or health plan resources. It is vitally important to document these determinants and code them when applicable to help identify patients who may qualify for resources.
