SDOH ICD-10-CM codes (Z55.- to Z65.-) are for supplemental reporting purposes. These codes may reflect self-reported information by the patient or could be based on documentation from other clinicians involved in the patient’s care, such as social workers, community health workers, case managers, or nurses. 


This information must be incorporated into the record by a managing provider and are not primary diagnosis codes. Please consult the ICD-10-CM code book for a complete listing of the codes.

  • What is the highest level of school you have completed?

Employment And Unemployment

  • Do you need or want help finding or keeping work?

Social Environment And Lifestyle

  • Do you exercise regularly?

  • Do you have family or community support for daily activities, including preparing meals, shopping, bathing, and managing finances?

  • Do you feel lonely or isolated?

  • In the last month, have you consumed alcoholic drinks, used tobacco products, or used drugs outside your prescribed medications?

  • How often do you feel down, depressed, or hopeless?

Additional Concerns

  • Do you feel safe at home?

  • Do you regularly feel stressed at home?
