Genuine Health ACO

 

Like all accountable care organizations, Genuine Health ACO is made up of providers that serve a defined population of “fee for service” Medicare patients. Currently, Genuine Health ACO has nearly 100 participating physicians, but we’re rapidly expanding our provider head count.

 

Genuine Health ACO operates under Track 1+ of CMS’ Medicare Shared Savings Program (MSSP), which qualifies our ACO as an “Advanced Alternative Payment Model,” or APM. As such, we bear financial risk for the cost of caring for the Medicare patients who are part of Genuine Health ACO.

Genuine Health MSO

 

Genuine Health’s management service organization (MSO) includes a cohort of primary care physicians who collectively have thousands of patients enrolled in the leading Medicare Advantage plans.

 

As is the case with our ACO, we bear financial risk for the cost of caring for our physicians’ patients who are enrolled in Medicare Advantage plans. In addition, for these doctors, we apply our advanced analytics tools to create individualized care plans for each of patient, including built-in follow up protocols that maximize adherence to the care plan.

 

 

Genuine Health at Home

 

Successful patient transition from hospital to home requires engaging patients and their families in the discharge planning process. It also requires on-site clinical assistance.

 

Genuine Health at Home provides this crucial bridge and works to ensure the transition from hospital to home is a safe one. From reviewing discharge care instructions, to monitoring prescription drug usage, to evaluating health status for signs of complications, the team keeps patients well once they’re back at home. This vital step in the transition process prevents readmissions and lowers overall healthcare costs. 

 

Genuine Health at Home is also capitalizing on evidence that home-based care can be more effective than care in a hospital setting, not only after discharge, but also for patients with chronic conditions. These patients account for up to half of healthcare delivery costs. So by addressing their needs more affordably – at home – patients fare better, and our physicians participate in the cost savings.
